Our office maintains a chart of all your medical information. This record is confidential.
No information in that record will be released to anyone without your written permission.

If you need reports sent to someone, you must let us know in writing. If you move and want to transfer you records, you may receive a copy from us.

However, it is easy to lose records in the confusion of a move. Usually, people find a new physician in their new home and that physician requests copies of your medical record.

We will be happy to send a copy of your medical record to your new physician after you sign a release.
